Good Faith Estimate

Your Right to Receive a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your healthcare services may cost.

If you do not have health insurance, or you have insurance but do not plan to use it to pay for your laboratory services, federal law generally gives you the right to receive a Good Faith Estimate of expected charges.

When You Can Receive an Estimate

  • If you request an estimate before scheduling: M1 will provide the Good Faith Estimate within 3 business days as required by applicable federal requirements.
  • If services are scheduled 3–9 business days in advance: M1 will provide the Good Faith Estimate within 1 business day after scheduling as required by applicable federal requirements.
  • If services are scheduled 10 or more business days in advance: M1 will provide the Good Faith Estimate within 3 business days after scheduling as required by applicable federal requirements.

A Good Faith Estimate is not a bill and does not require the patient to obtain the listed services from M1.

The final charges may differ if additional or unexpected items or services become necessary.

If Your Bill Is Higher Than Your Estimate

If you receive a bill from a provider or facility that is at least $400 more than that provider or facility’s applicable Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process.

Keep copies of your Good Faith Estimate and your final bill.
